At CDS, we have assembled a team of
qualified and Certified Medication
Technicians who are thoroughly vetted,
examined and trained before they are
assigned to work in any home.
This vetting process encompasses many
aspects, the least of which is a thorough
background check. On routine basis, the
delegating nurse offers refresher training
sessions based on changing medical
needs of the individual.
CDS’ staff persons are also frequently
trained by certified trainers to keep in
compliance with the ever-changing
regulatory terrain of DDA as well as
equipping our staff persons with the
necessary skills to deal with unexpected
situations that may arise while at work.
Services
Residential Services
CDS pursues homes in communities based on the
preferences of the individuals. CDS encourages
individuals to realize their own needs and in making these
choices. CDS realizes that, independence in expressing
their choice is fundamental to rights and human dignity.
Individuals at CDS are empowered to have control and
choice in whatever they do. These choices range from
preference of a health care professional, mode of
transportation in the community, religion, employment,
etc. Each home is equipped with an assigned vehicle for
individuals to participate in activities of their choice and
for medical appointments.
CDS provides community-based services in the residential
setting. While the range of community residential service
options available to the individual may be limited by
resources and/or individual circumstances (forensic
involvement), all services are designed to maximize each
individual’s potential. The residential home will house no
more than three individuals per licensed unit. Individuals
will be provided supports in accordance to their
individualized preferences and needs identified in the
Person-Centered Plan.

Day Services
Community Development Services are premised on the paradigm that all
individuals with unique abilities can thrive when given the opportunity,
training, and supports that build and promote an individual’s strength. The
Community Development Service model is meant to maximize individual
independence and reduce level of service need.
Community Development Services are forged through a person-centered
planning process which is delivered in line with the individual’s
employment/volunteer aspirations. In order to promote positive growth
and/or assist individuals in developing the skills and social supports necessary
to gain, retain or advance in employment, CDS places an emphasis on
making sure that services are specific, individualized and goal-oriented.
CDS conducts annual assessments to measure the efficacy of activities,
special assistance, support and education to help individuals whose age,
unique ability or circumstances currently limit their ability to be employed
and/or participate in activities in their communities. Programs are structured
to promote self-determination/self-advocacy, self–advocacy training,
workshops and classes, peer mentoring, volunteer activities and activities that
promote health and socialization. CDS is responsible for providing/arranging
transportation to support Community Development Services activities.
Employment Services
Carpe Diem Semper, Inc. provides employment services to individuals with
unique abilities under several sub categories such as Career Exploration,
Discovery, Job Development, Follow Along Job Supports, Ongoing Job
Supports, Self-Employment Development Supports and Co-worker
Employment Supports. Depending on the strengths and needs in accordance
with stipulations in any individual’s Person-Centered Plan, CDS determines
the most appropriate modality of service.
